As a patient, I am consenting to allow TeleCare Solutions and their affiliates, doctors, and medical assistants to facilitate my medical care using telemedicine services whom may provide services from different location thus, requiring my ability to be forthcoming with accurate information with regards to my medical history, lifestyle, and care.

  4. You are informed about the potential risks and benefits associated with the proposed medical services. You have the right to ask questions and seek clarification on any aspect of your care, and our team is here to provide that information.

  5. I acknowledge that the services provided by TeleCare Solutions is not a guaranteed cure, and if needed a refund will be offered if an in-person evaluation or referral is determined to be better solution.

  6. I understand that it is my responsibility to alert my physician if I experience a negative reaction to any treatment provided by TeleCare Solutions. If necessary, call 911 or go to the nearest emergency room.

  7. TeleCare Solutions is HIPPA compliant, however due to unauthorized viewers, you recognized that electronic transmission may be compromised at no fault of TeleCare Solutions due to illegal access beyond our security precautions.

  8. I hereby, acknowledge that TeleCare Solutions and affiliates are released from any liability which may occur through the unauthorized use of audio, video, or other electronic transmission.

  9. You agreed to be responsible for any costs incurred during your treatment. The initial fee only covers the initial evaluation and I am responsible for any and all additional cost of medication, if needed.

  10. I consent that I have read, or have have had the consents read to me, and I agree to all the information here.
